Hi, I need to replace this part on my 2000 Montero Sport, but not sure what it is called and where to get it (and if I should attempt replacing it myself).

It's the Lift Gate Exterior Trim Finish Panel, MSRP $305.12. The cheapest place I have found to order new Mistubishi OEM parts is www.jnztuning.net ($236.47). You can look on www.car-part.com, a huge online database of parts recyclers in the US for your specific part from across the country at widely varying prices. Hope this helps.
